REQUIREMENTS FOR U.S. CITIZENS TO ENTER LEBANON

TOURIST TRAVEL
U.S. Passport must be valid 3 months beyond intended stay
Tickets and Documents for return or onward travel
Visa Required - can also be obtained directly upon arrival at the Beirut Airport or any other port of entry
Vaccinations - International Certificate of Vaccination for Yellow Fever required if arriving from infected area within 5 days
The passport and documents must not contain evidence of prior or intended travel to Israel.

BUSINESS TRAVEL
U.S. Passport must be valid 3 months beyond intended stay

APPLICATION FORM (PRINT OR TYPE)
PASSPORT (VALID FOR A MINIMUM OF SIX MONTH)
ONE RECENT PASSPORT'S SIZE PICTURE
MONEY ORDER FOR THE AMOUNT REQUIRED
ONE OF THE FOLLOWING:
For Lebanese-American Applicants: Enclose a photocopy of your Lebanese I.D. Card or Extract of Civil Status.
For Wifes of Lebanese Nationals: A photocopy of the marriage certificate and a copy of the spouse's Lebanese I.D.
For Minors: A notarized parents' consent form (signed by both parents) agreeing to the issuance of the visa or a custody court order allowing explicitly the minor to leave the USA in case of divorced or separated spouses. Photocopies of the minor's birth certificate and of the parents' I.D.
For Business Travelelrs & Press: A letter from employer or host in Lebanon stating purpose of visit and full address there.
6. A Certified self-addressed stamped envelope or a pre-paid Express Mail.
